Job Description:
Responsible for implementing all aspects of one student's Individualized Education Program (IEP), Behavior Intervention Plan (BIP), and Health Care Plan (HCP). Requires a high degree of flexibility, consistency, and professional judgment to ensure the student's safety, access to instruction, and full participation across the school day and various learning environments. Performs other related duties as assigned or requested.

Position Specific Information (if Applicable):
Responsibilities:
Implement all academic accommodations, modifications as written in the student's current IEP.
Execute the student's Behavior Intervention Plan (BIP) and crisis management strategies consistently across all settings, including instruction, transitions, and unstructured time.
Collect and accurately record all required data on academic, behavioral, social, and functional goals, reporting progress to the Special Education Teacher daily/weekly.
Access and Inclusion Support:
Provide direct 1:1 support to the student in the general education classroom, special education setting, and all non-instructional environments (e.g., lunch, recess, assemblies, field trips).
Individualized Plan Implementation
  • Facilitate peer interaction and social skills development by modeling appropriate social behaviors and supporting the student's successful participation in group activities.
  • Physically assist the student with transitions between classes, activities, and locations within the school building and community.

Safety, Health, and Personal Care:
  • Maintain a line of sight/close proximity with the student at all times to ensure immediate response to any safety or health needs.
  • Provide personal care assistance, which may include feeding, toileting, hygiene, dressing, and positioning, in a dignified and professional manner.
  • Implement the student's Health Care Plan (HCP), which may involve delegated nursing tasks such as medication administration, seizure protocol implementation, or monitoring of medical equipment, following all required training.
