Position - Case Manager for Lake Park Residential
Regional Health Systems is Northwest Indiana's largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children-both uninsured and insured-integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group.
Position Description:
We are seeking a dedicated and compassionate Case Manager to join our team at our Lake Station location. The ideal candidate will play a critical role in supporting individuals and families by providing guidance, resources, and advocacy. This position requires a strong understanding of behavioral health, child welfare, and crisis management to effectively assist clients in navigating their challenges and achieving their goals.
Essential Functions:
  • Conduct comprehensive assessments to identify client needs and develop personalized service plans.
  • Provide ongoing support and case management services to clients, ensuring they have access to necessary resources.
  • Collaborate with multidisciplinary teams, including healthcare providers, social workers, and community organizations.
  • Facilitate communication between clients and service providers to ensure continuity of care.
  • Monitor client progress and adjust service plans as needed to promote positive outcomes.
  • Utilize motivational interviewing techniques to empower clients in their decision-making processes.
  • Maintain accurate documentation of client interactions, progress notes, and case files in compliance with organizational policies.
  • Stay informed about community resources, including Section 8 housing assistance and workforce development programs.

Pay is based on years of experience:
$17.93 - $23.31 / hour
