Job description

Responsible for the clinical and administrative oversight, supervision and support to the Integrated Health Home (IHH) Team that provides community-based services and supports to adults residing in the community who are living with a severe and persistent mental illness.

Must maintain knowledge in best practices for treating clients with behavioral health and substance use disorders through a comprehensive, integrated care model that includes outreach, engagement, treatment, coordination of care, and information and referral. Responsibilities include effectively leading an interdisciplinary team to ensure that all clients receive comprehensive, quality care and services to promote recovery, rehabilitation, and well-being. Clinical and supervisory experience is required.

MAIN DUTIES AND RESPONSIBILITIES:

  • Maintains current knowledge of diagnostic, intervention and treatment strategies that are considered best practice for adults experiencing severe and persistent mental illness and substance use disorders.
  • Facilitates admissions and discharges of clients on team.
  • Assesses to determine program eligibility, engagement in treatment, appropriate level of care, coordination of care, and discharge planning.
  • Assigns client caseloads and ensures all clients receive care in accordance with individualized treatment plan. Coordinates coverage as needed.
  • Facilitates team morning meeting and clinical meeting; ensures collaboration and coordination of care.
  • Supervises staff in providing education and support to clients and their families, regarding diagnosis, treatment options and recommendations, and self-care techniques.
  • Ensures coordination of client care with practitioners involved in client care, including psychiatrist, primary care physician and other medical or clinical practitioners. Facilitates scheduling of appointments.
  • Provides daily, ongoing supervision and support to staff and assists with outreach services as needed including conducting risk assessments and implementing crisis intervention strategies/safety planning to maintain client safety and promote well-being.
  • Escalates care as needed.
  • Assists staff to identify client and family needs (housing, education, vocational, social/leisure, financial, food, transportation, medical, substance, etc.) and make appropriate referrals as needed.
  • Monitors and ensures payor sources for qualifying services, e.g., Medicaid, Title XX, etc. and ensures that authorization processes are in place and correct billing codes/charges are used.
  • Ensures the collection of required data for BHDDH /Gateway and submission of reports within required timeframes.
  • Provides other reports, analyses of data, etc. as requested. Hires, orients, trains, and evaluates program staff according to Gateway Healthcare policy.
  • Provides individual and group supervision to team members and maintains supervision records in accordance with BHDDH regulations and Gateway policies and procedures.
  • Monitors and addresses concerns with staff performance.
  • Assists the finance department in resolving authorization and billing issues.
  • Monitors and ensures compliance with BHDDH regulations and Brown University Health/Gateway policies &procedures.
  • Ensures staff complete documentation in accordance with Gateway's policies and procedures.
  • Monitors program budget(s) to ensure compliance with revenue and expense targets.
  • Completes administrative tasks of approving payroll, mileage, benefit time, etc.
  • Conducts self in a professional and ethical manner in keeping with Gateway Healthcare policy and professional guidelines on ethical conduct.
  • Performs other duties and responsibilities as assigned or needed.


QUALIFICATION REQUIREMENTS:

  • Master's degree in related field or RN or licensed independent practitioner (LICSW, LMHC, LMFT, LCDP) preferred
  • At least 2 years experience supervising program staff.
  • Demonstrated expertise with adults with behavioral health needs.

NECESSARY SPECIAL REQUIREMENTS:

  • Knowledge of behavioral health disorders, substance use disorders, and chemical dependency treatment in accordance with the core functions of counseling.
  • Knowledge of community resources for clients with behavioral health/substance use disorders.
  • Valid driver's license and use of car during normal working hours or other means of transportation in order to perform the major functions of the job.
  • Automobile liability insurance coverage for vehicle being used in accordance with Center requirements.

JOB KNOWLEDGE, SKILL, AND ABILITY:

  • Ability to effectively present information to supervisor, other members of the treatment staff, and clients and their family members.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Demonstrated capacity to maintain an effective working relationship with members of a multi-disciplinary treatment team.
  • Ability to successfully complete all required training and certifications.
  • Oral and written communication skills, including speech, grammar, spelling, and legibility are at sufficient level to ensure effective and appropriate communication.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals either manually or by use of a calculator. ETHICS
  • Complies with Gateway Healthcare ethics and codes of conduct in all contacts with clients, co-workers and the community.
  • Uses and accesses information in client's health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc. policies, rules, and procedures as adopted by Gateway Healthcare Inc
  • Observe all agency policies and procedures.

  • Must be able to appreciate the sensitive/private nature and legal status of information about individual clients obtained, maintained, or used by Gateway Healthcare Inc. and to not access, use, disclose, or request such information about a client unless necessary to the performance of his/her job.

Pay Range:

$64,480.00-$106,412.80
