• Perform comprehensive mental health assessments, factoring in accompanying medical conditions that may impact psychological well-being.
• Develop and implement individualized treatment plans that encompass both psychiatric and medical needs, utilizing evidence-based practices.
• Facilitate one-on-one and group therapy sessions, customizing approaches to fit the unique circumstances and histories of each patient.
• Work collaboratively with medical professionals to ensure seamless care coordination and treatment efficacy.
• Provide crisis intervention and immediate support for patients experiencing mental health emergencies.
• Educate patients on the interplay between mental and physical health, empowering them to take charge of their overall well-being.
• Maintain thorough and confidential documentation in accordance with legal and ethical standards, utilizing electronic health record systems.
• Engage actively in team meetings, case conferences, and interdisciplinary planning sessions to enhance treatment strategies.

The primary focus of this role includes working with individuals grappling with a range of co-occurring conditions such as autoimmune disorders, chronic pain, and substance use disorders. You will play a vital role in a patient’s journey to recovery, fostering a supportive environment for them to confront and navigate various challenges.

• Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.
• Demonstrated experience in addressing the mental health needs of individuals with co-occurring medical conditions.
• Strong interpersonal and communication skills, with the ability to connect with patients empathetically and effectively.
• Commitment to maintaining professional boundaries and confidentiality in all interactions.
• Proficiency in utilizing electronic health record systems and telehealth technologies.

Desirable candidates will possess experience in integrated care settings, demonstrating a robust understanding of how medical and mental health services can complement each other to improve patient outcomes. Familiarity with evidence-based therapeutic modalities and crisis intervention techniques is also preferred.
