Job description

Enso Housing Case Manager

Reports To: Community Case Management Supervisor

Department: Enso Residences                      

FLSA Status: Non-Exempt                                                                                                                              

Shift/Schedule: Mon-Fri 8am-4:30pm          


Position Summary

Enso Residences is a Permanent Supportive Housing Development designed to support at-risk and chronically homeless individuals toward stable housing in the community. The Enso Housing Case Managers of Enso Residences will work with the Lead Case Manager and Community Case Management Supervisor to provide on-site person-centered, trauma-informed care to all 60 residents in the building. The position will hold a caseload of up to 20 residents to support them with symptom management, skill-building, maintaining housing, accessing healthcare, crisis management, and benefits coordination.

Essential Functions:

  • Engage in community building activities such as groups, community meals, and events
  • Support clients with lease signing, move in, and coordination with property management
  • Complete home visits to assess client’s living environment and provide support as needed to help them maintain stable housing
  • Connect clients to community resources including healthcare, employment, crisis services, benefits
  • Facilitate wraparound recovery plan meetings every 6 months with client and other service providers
  • Conduct screenings and assessments to evaluate individual client need
  • Assist clients in creating goals and implementing action plans to achieve them
  • Document all client interactions by entering progress notes into EHR

Qualifications

  • A Master’s Degree in health/social services, preferred
  • 2+ years of experience in Human Services field, required
  • SAC or SAC-IT, preferred
  • CCS experience and certified, preferred
  • Bi-lingual, preferred
  • Valid Driver's License
  • Pass a criminal background check and be free of offenses prohibiting employment under HSF12 at the time of employment, and annually thereafter.  

Physical Requirements

 
  • Lift and carry objects such as boxes, bags, or other materials up to 25 lbs
  • Must be able to stand or sit for extended periods of time
