Job Description:
• Provide high quality, individualizes patient care in collaboration with our nursing team
• Take initiative while assisting and maintaining patient care
• Assist the resident in activities necessary as per individualized care planning
• Transport Elders to and from activities
• Perform evaluation and reporting
• Provide resident and family with emotional support individualized care planning
• Receive/give reports to team members/team leaders to appropriate professional staff
Candidate Requirements:
• Valid NYS Certified Nursing Assistant License (CNA) - Required
• Minimum of 1 year experience working with Elders - Preferred
• Ability to work in a team oriented work group
• Proficient in various computer programs and Medical Records Documentation system
