Providence at Home is a growing division of Providence Life Services, which provides a continuum of healthcare, focused on senior living. Providence Hospice is seeking a RN Hospice Case Manager, Full Time in St. John, IN and throughout Lake County . We are looking for a compassionate RN to join our established, experienced hospice team. We provide exceptional patient centered care to patients and families facing end of life. $5,000 retention bonus now being offered . Retention Bonus is paid out in increments when milestone hours are met.
Schedule Available: Monday - Friday. This position requires (1) on call night shift weekly.

Requirements
  • Active Registered Nurse (RN) License in the state of Indiana.
  • At least 1 year of RN experience, preferably in Hospice or Home Health.
  • Drivers License in good standing.
  • Reliable transportation.
