Responsibilities:
- Coordinate and manage comprehensive care plans tailored to each patient's unique needs, ensuring their comfort and dignity.
- Collaborate closely with interdisciplinary teams to deliver holistic care, addressing physical, emotional, and spiritual well-being.
- Conduct regular assessments and adjust care plans as needed, keeping the patient's best interest at heart.
- Educate patients and their families on care processes, empowering them with knowledge and confidence in their journey.
- Serve as a compassionate advocate for patients, ensuring their voices are heard and respected in all care decisions.
- Maintain accurate and timely documentation of patient care activities, supporting seamless communication across teams.
- Participate actively in team meetings, sharing insights and strategies to enhance patient care and team collaboration.
Qualifications:
- Current Registered Nurse (RN) license in the state of New Jersey or Pennsylvania.
