Compensation:

$53 - $56 hourly


Responsibilities:
  • Coordinate and manage comprehensive care plans tailored to each patient's unique needs, ensuring their comfort and dignity.
  • Collaborate closely with interdisciplinary teams to deliver holistic care, addressing physical, emotional, and spiritual well-being.
  • Conduct regular assessments and adjust care plans as needed, keeping the patient's best interest at heart.
  • Educate patients and their families on care processes, empowering them with knowledge and confidence in their journey.
  • Serve as a compassionate advocate for patients, ensuring their voices are heard and respected in all care decisions.
  • Maintain accurate and timely documentation of patient care activities, supporting seamless communication across teams.
  • Participate actively in team meetings, sharing insights and strategies to enhance patient care and team collaboration.

Qualifications:
  • Current Registered Nurse (RN) license in the state of New Jersey or Pennsylvania.
  • Experience in hospice or palliative care, demonstrating a deep understanding of end-of-life care needs.
  • Ability to develop and manage individualized care plans, ensuring patient comfort and dignity.
  • Proven track record of effective collaboration with interdisciplinary teams to deliver holistic care.
  • Strong communication skills to educate and empower patients and their families.
  • Compassionate advocacy skills to ensure patients' voices are heard and respected in care decisions.
  • Proficiency in maintaining accurate and timely documentation to support seamless team communication.
