Job description

Title: LPN - Medical Surgical (Discharge Lounge)

Location: Rochester, New York

Duration: 13 Weeks

Schedule: 4x10-Hour Shifts | 4 Days Per Week

Shift Timing: 10:00 AM - 7:00 PM

Position Overview:

"Navitas Healthcare, LLC" is seeking an experienced LPN - Medical Surgical (Discharge Lounge) to provide patient-centered care in an acute care environment. The ideal candidate will be comfortable managing patient assignments with RN support and assisting patients through the discharge process while maintaining high-quality clinical care.

Primary Responsibilities:

  • Provide direct patient care in a Medical-Surgical setting

  • Monitor and document vital signs and patient assessments

  • Administer medications and treatments as prescribed

  • Assist with discharge planning and patient education

  • Monitor intake and output

  • Perform blood glucose monitoring

  • Manage oxygen therapy and telemetry monitoring

  • Assist with feeding tube management and Foley catheter care

  • Perform specimen collection and labeling

  • Provide wound care and drain management

  • Communicate patient condition changes to RN and physicians

  • Follow infection control and isolation protocols

  • Float to other units within skill set as required

Required Qualifications:

  • Active New York State LPN License required

  • BLS Certification required

  • Acute Care experience within the last 2 years

  • EPIC experience required

  • Ability to manage patient assignments with RN support
