Position Overview

Brighter Life Social Work is actively seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in Colorado Springs, CO. This full-time, remote position focuses on providing essential support and counseling to individuals and families at our Hospice Center, ensuring they receive comprehensive care during critical phases of life.

Work Environment

This role offers a unique remote work setting, allowing for flexibility while maintaining a collaborative approach to service delivery. You will be connecting with clients via virtual platforms and phone communications while occasionally meeting in-person for assessments and support when necessary.

Key Responsibilities
  • Conduct thorough assessments focusing on the social, emotional, and environmental factors affecting clients, ensuring an insightful understanding of their situations.
  • Develop tailored service plans outlining specific goals and interventions, including periodic reviews to monitor progress and adjust as necessary.
  • Provide compassionate counseling to individuals and families facing various challenges, utilizing trauma-informed care principles to create a safe and supportive environment.
  • Collaborate directly with clients to ensure they are connected with vital community resources, enhancing their ability to navigate complex social systems.
  • Engage in crisis intervention initiatives, performing risk assessments and developing safety plans to support those in urgent need.
  • Work in tandem with community agencies and organizations to coordinate efforts and advocate effectively on behalf of clients, thus promoting access to critical services.
  • Participate in community outreach initiatives aimed at increasing awareness of the services our organization offers, thereby engaging more clients and stakeholders.
  • Actively participate in case conferences, team meetings, and ongoing staff training to continually improve service delivery and professional development.
Patient Population / Client Focus

The primary clientele includes individuals and families navigating the sensitive context of hospice care, including end-of-life considerations. This role is pivotal in providing emotional support and resource navigation for clients facing grief, loss, and terminal illnesses. With a solid understanding of the complexities surrounding hospice services, the LMSW will play a crucial role in ensuring clients receive compassionate and comprehensive care.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited program.
  • An active state license as a Licensed Master Social Worker (LMSW).
  • Strong knowledge of social and economic justice issues, trauma-informed care, and local community resources.
  • Exceptional communication and interpersonal skills, demonstrating cultural competence and the ability to connect with diverse populations.
Preferred Experience
  • At least two years of relevant experience working in a social service agency or related environment preferred.
  • Familiarity with end-of-life care issues, palliative care, and grief counseling is advantageous.
