Job description

You will be part of an interdisciplinary care team of social workers, chaplains, nurses, physicians, and certified nursing assistants all working together to improve people's everyday lives when they need help the most. You will be an advocate by providing education and resources to patients and their families, being out on the road and seeing our patients/families in their personal living environments in this beautiful rural community.
The Social Worker will work with an exemplary and robust interdisciplinary team focused on providing quality care to patients in the community through education, resources, and care that honors the goals and wishes of each person.
Who Should Apply?
