JOB SUMMARY:
The Medical Social Work Care Manager collaborates with the Clinical Care Coordinator (C3) and other members of the care management and health care teams, utilizes professional Social Work skills in accordance with social work practice standards to: assist the patient in achieving optimal health; assure appropriate use of health care resources; facilitate/coordinate timely care along the continuum; decrease unnecessary variances in care through proactive care management; develop a safe and timely discharge plan in accordance with the Joint Commission and CMS Guidelines, each patient will be ensured a planned program of continuing care which meets his/her discharge needs and to ensure a smooth hospital stay with a seamless transition to the next level of care following discharge from GHS.
SKILLS/CERT/LICENSES:
Preferred: Licensed Medical Social Worker
Case Manager Certification/Accredited Case Manager
EDUCATION:
Master's Degree in Social Work (MSW).
EXPERIENCE:
Prefer: Experience in a hospital environment.
Previous experience providing social work/case management services.
