How you’ll make an impact in this role
  • Develop care plans for patients based on assessment findings and consideration of needs and preferences.
  • Assesses complex emotional, psychological and physical issues to understand what the patient requires and how social work can provide support.
  • Advocate for patients' needs and ensures the medical system respects their rights and caters to their preferences.
  • Maintain ongoing dialog with leadership and care transition team members to ensure effective implementation and reevaluation of care plan.

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • One or more of the following required:
    • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
    • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
