Your future role at a glance

Location: Milwaukee, WI

Facility: St. Francis

Department/Specialty: Case Management

Schedule: Rotating | Full-time

How you'll make an impact in this role

Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

  • Design comprehensive, personalized care pathways that bridge the gap between hospital and home, ensuring every patient's values and family preferences are at the heart of their recovery.
  • Champion patient needs by identifying and removing systemic barriers, ensuring every individual has the resources and advocacy required for a seamless transition.
  • Orchestrate clinical outcomes by maintaining a continuous, high-level dialogue with the multidisciplinary team to ensure care plans are executed flawlessly.
  • Safeguard the integrity of the patient journey by overseeing clinical documentation and evaluating service appropriateness to guarantee the highest standard of post-discharge care.
What minimum qualifications you'll need

Licensure / Certification / Registration:

  • Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required. 
  • Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date required.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. 

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
