Position Overview

Vow Venture Therapies is actively seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team remotely, focusing on providing support in our Hospice Center located in Allentown, Pennsylvania. Key Responsibilities
  • Conduct thorough assessments for clients coping with end-of-life issues, identifying their social, emotional, and environmental challenges.
  • Develop and implement personalized service plans that include clear goals, interventions, and methods for monitoring progress.
  • Provide empathetic counseling and support to individuals and families, guiding them through their emotional landscapes while ensuring their dignity and comfort.
  • Collaborate with clients to create actionable service plans that connect them with vital community resources and services.
  • Utilize crisis intervention strategies and perform risk assessments to ensure the safety and well-being of clients and their families.
  • Engage with community agencies and other organizations to advocate on behalf of clients, ensuring they receive comprehensive care.
  • Participate in outreach activities designed to raise awareness of available services and build rapport within the community.
  • Contribute to case conferences and participate in team meetings aimed at improving service efficacy.
Patient Population / Client Focus

Your work will primarily revolve around patients in hospice care, focusing on individuals experiencing terminal illnesses and their families. Qualifications
  • A Master's degree in Social Work from an accredited institution is required.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
