• Conduct thorough assessments for clients coping with end-of-life issues, identifying their social, emotional, and environmental challenges.
• Develop and implement personalized service plans that include clear goals, interventions, and methods for monitoring progress.
• Provide empathetic counseling and support to individuals and families, guiding them through their emotional landscapes while ensuring their dignity and comfort.
• Collaborate with clients to create actionable service plans that connect them with vital community resources and services.
• Utilize crisis intervention strategies and perform risk assessments to ensure the safety and well-being of clients and their families.
• Engage with community agencies and other organizations to advocate on behalf of clients, ensuring they receive comprehensive care.
• Participate in outreach activities designed to raise awareness of available services and build rapport within the community.
• Contribute to case conferences and participate in team meetings aimed at improving service efficacy.

Your work will primarily revolve around patients in hospice care, focusing on individuals experiencing terminal illnesses and their families. You will be key in addressing their unique emotional and social needs, ensuring comfort, dignity, and support during this crucial time in their lives. This role emphasizes building trusting relationships that enable open communication and provide vital support systems, allowing clients to navigate their experiences in a compassionate environment.

• A Master's degree in Social Work from an accredited institution is required.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
• Demonstrated knowledge of hospice and palliative care principles, social and economic justice issues, and community resources.
• Strong communication skills with a focus on cultural competence and sensitivity.
• Proficient problem-solving capabilities, especially in high-stress situations.

• Previous experience in a social service agency, particularly in hospice or end-of-life care settings, is highly preferred.
• Familiarity with trauma-informed care practices and community resource networks.
• Experience in developing and executing community outreach programs.
