Social Services Director

Full-Time | Exempt | Department Head

We are seeking an experienced and compassionate Social Services Director to lead all social services operations within the facility. This role is responsible for ensuring residents’ psychosocial, emotional, and mental health needs are identified and met while maintaining compliance with federal and state regulations and professional social work standards. This position is ideal for a strong social services leader who values advocacy, regulatory compliance, interdisciplinary collaboration, and resident-centered care.

Responsibilities

• Plan, organize, and direct all Social Services Department operations
• Ensure compliance with federal, state, and facility regulations and NASW ethical standards
• Advocate for resident rights, dignity, and psychosocial well-being
• Complete and oversee psychosocial assessments, care planning, and documentation
• Coordinate PASRR processes, advance directives, and resident rights notifications
• Oversee discharge planning and coordination with community resources and agencies
• Address resident and family grievances and ensure appropriate resolution
• Provide support for residents and families during admissions, transfers, and transitions
• Coordinate mental health, hospice, and palliative care referrals
• Lead social services participation in interdisciplinary team meetings
• Maintain survey readiness and participate in surveys, audits, and plans of correction
• Supervise, train, evaluate, and discipline Social Services staff as needed
• Develop and maintain a quality assurance program for Social Services
• Provide staff education related to psychosocial needs, resident rights, and abuse prevention

Qualifications

• Bachelor’s degree in social work, psychology, sociology, or related field required
• Master’s degree in Social Work preferred
• Minimum three (3) years of supervised social work experience in a healthcare setting preferred
• Long-term care experience strongly preferred
• Strong knowledge of resident rights, discharge planning, and regulatory requirements
• Excellent communication, documentation, and leadership skills
• Ability to manage sensitive situations with professionalism and discretion

Physical Requirements

• Office and facility-based work throughout the day
• Standing, walking, and moving throughout the facility as needed
• Exposure to clinical and resident care environments
