Job description

Elaine Ellis Center of Health is looking for a Clinical Social Worker to join our care team. This part time patient advocate position is responsible for enhancing the overall quality of care with the medical, behavioral and dental services provided at EECH through patient support, assistance, collaboration and communication. As a federally qualified healthcare center EECH has a strong patient advocacy need to assist our patients in reducing barriers to accessing healthcare and overcoming social determinants of health. Successful candidates will be licensed in DC and Maryland and meet credentialing requirements with insurance payers.


SPECIFIC DUTIES AND RESPONSIBILITIES:

· Provide ongoing support and expertise through comprehensive assessment, planning, implementation and evaluation of the patients’ overall needs.

· Facilitate patient follow up with referrals and compliance with recommended appointments.

· Create a comprehensive referral base and establish professional relationships with referral specialists to ensure smooth and consistent communication.

· Provide case management as the patient progresses through the continuum of care in the form of referrals, access to community resources and follow-up care as needed.

· Coordinate the integration of social services/case management functions into the patient care process with other providers, external service agencies and healthcare facilities.

· Support patients, families and caregivers in self-management, self-efficacy and behavioral changes.

· Assist patients in getting help needed from government agencies and other community resources.

· Coordinate chronic care management for the appropriate patients.

· Conduct follow-up outreach for post ED visits and managed care organization (MCO) referrals utilizing CRISP and MCO panels.

· Collaborate with clinical staff in the development and execution of the plan of care and achievement of goals.

· Communicate with patients via telehealth, as appropriate.

· Liaison between insurance companies, providers and patients for preauthorization, referrals; and keeping current with insurance portals and available resources.

· Assisting patients, as needed, with insurance eligibility needs.

· Actively participate in team training for patient centered care coordination.

· Participate in the development and implementation of policies and procedures that impact special cases or patient needs.

· Maintain accurate and complete patient records.

· Conduct utilization reviews for the targeted population.

· Perform other duties and assist with special projects as assigned.

· Represents the Center at professional organization meetings and training.
