About the Role

The Social Services Assistant plays a vital role in supporting the delivery of compassionate and effective care to residents. The Social Services Staff provide medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident. Additionally, the role involves maintaining accurate records and utilizing basic computer skills to document care activities and communicate with the care team. Ultimately, the Social Services Assistant contributes to enhancing the quality of life for residents by providing attentive, respectful, and professional support.

Job Responsibilities

  • Assesses each resident's needs at admission and thereafter with input from the resident, resident's representative, and interested family members and develops the social service care plan.
  • Help residents and spouses utilize community resources through referral when the services needed are not provided by the facility.
  • Reviews care plans and write progress notes of a resident's response to the Social Service care plan when required by facility policies. Completes appropriate assessments, social history and social service portions of MDS.
  • Provides advice and appropriate referrals to minimize social and economic obstacles to discharge, and coordinates discharge planning communications and documentation.
  • Communicate effectively with residents, families, and the care team to ensure coordinated support.
  • Support the overall well-being of residents by responding to their needs with patience and empathy.

Minimum Qualifications

  • Indiana board certification in social work or a bachelor's degree in social work or related field.
  • Must have at least (1) year of experience in a health care setting working directly with individuals. If no degree, then you must have a high school diploma or its equivalent and have completed, or will complete within 6 months, the forty-eight (48) hour social services course approved by the State.
  • Basic training or certification in patient care or a related field.
  • Basic computer literacy, including familiarity with electronic health records or similar software.

Preferred Qualifications

  • Previous experience working in a healthcare or social services environment.
  • Experience with specialized healthcare software or electronic medical records.
  • Strong interpersonal and communication skills developed through prior roles.
