Part time Registered Nurse Case Manager Anticipated Territory : Redwood City-Sunnyvale Area Schedule: Saturday/Sunday What you'll do: Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including: Conduct Start of Care (SOC) Admissions - OASIS Conduct follow ups, evaluation visits as needed in designated territory Make home visits to patients in designated territory Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures Monitor clients' conditions, reporting changes to Clinical team Collaborate with physicians and agency staff to deliver the best care possible Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
