Job description

RN Hospice On Call Position for Triage and Visit Support - Monday - Thursday, 11p-8am

Purpose of Job

This position is responsible for the delivery of individualized patient care through the nursing process of assessment, planning, implementation and evaluation in accordance with the Standards of Nursing Care and Practice as defined by Ridgeview Medical Center, the Minnesota Nurse Practice Standards Act and the ANA Standards of Clinical Nursing Practice.

Job Functions

  1. Conducts admission and recertification as ordered by the physician. Completes supporting documentation in an accurate and timely manner.
  2. Coordinates requests/referrals according to patient need and physician order.
  3. Performs supervisory visits and evaluates performance through direct or indirect observation of aides providing care.
  4. Advises and educates patients, family members and significant others on health maintenance/status, disease prevention and/or end-of-life care.
  5. Serves as a patient advocate in seeking appropriate community resources to meet patient need.
  6. Coordinates with and updates the Case Manager in a timely manner.
  7. Completes SOC and Quarterly audits to meet the regulatory requirements as requested by manager/PCC.
  8. Demonstrates competencies in the specialty area of Home Health.
  9. Performs case management functions under the guidance of a supervisor or primary care clinician as assigned.
  10. Performs other duties as assigned.

Minimum Education/Work Experience

  • Graduate of an accredited school of nursing

Knowledge/Skills/Abilities

  • Demonstrate critical thinking and decision making
  • Ability to meet and maintain the necessary background checks as aligned with position functions
  • Ability to work and multi-task in a high stress and fast paced environment
  • Capable of making independent decisions with the use of good critical thinking and problem solving skills
  • Ability to work autonomously and as a team
  • Ability to respond appropriately to unpredictable situations and diverse patient populations
  • Ability to communicate in the English language for effective written and verbal correspondence in order to complete job functions as mentioned above

License/Certification

  • Current professional nurse licensure in Minnesota or if licensed in another state, a permit from the Minnesota Board of Nursing while the license application is in process
  • Valid MN driver's license and meets Ridgeview's insurability requirements
  • Current Healthcare Provider CPR certification

Preferred Qualifications

  • Bachelor's degree in Nursing
  • COS-C (only for skilled)
  • CHPN - Certification of Hospice and Palliative Nurse (only for hospice)
  • RN experience in acute/skilled facility or home health
