Qualifications:

EDUCATION and/or EXPERIENCE

  • The Physical Therapist must have completed training in accordance with minimum essential for physical therapy schools as prescribed by Council on Medical Education and Hospitals of American Medical Association. He/she must graduate from an AMA-APTA approved school of physical therapy. The Physical Therapist must be a member or eligible for membership in the American Physical Therapy Association or American Registry of Physical Therapy, and must hold active license for the state of Colorado.

CERTIFICATES, LICENSES, REGISTRATIONS

1. Current, active Physical Therapist licensure for Colorado or a permit from the licensing board while awaiting licensure examination.

2. Current CPR Certification.
