Position Summary

We are seeking a dynamic and experienced Director of Clinical Services (DCS) to lead our home health clinical operations in the Portland market. This role is responsible for ensuring the delivery of high-quality, compliant, and compassionate care while developing and mentoring clinical leaders and staff.

The DCS is a key member of the leadership team and plays a critical role in driving clinical excellence, regulatory compliance, and team engagement.

Key Responsibilities
  • Provide oversight of all clinical operations for the home health agency
  • Ensure compliance with federal, state, and local regulations (CMS, OASIS, etc.)
  • Lead and mentor clinical staff, including nurses, therapists, and clinical supervisors
  • Drive quality outcomes, patient satisfaction, and process improvements
  • Manage clinical performance metrics, audits, and surveys
  • Collaborate with the Executive Director and leadership team on strategic initiatives
  • Oversee care planning, case management, and coordination of services
  • Foster a culture of accountability, engagement, and continuous learning
Qualifications
  • Current Registered Nurse (RN) license in Oregon (or ability to obtain)
  • Minimum 2–3 years of leadership experience in home health or related healthcare setting
  • Strong knowledge of Medicare regulations, OASIS, and home health operations
  • Proven ability to lead, develop, and retain high-performing clinical teams
  • Excellent communication, organizational, and problem-solving skills
  • Passion for patient-centered care and team development
