🧑 Position Summary

As a Home Health Physical Therapist Assistant (PTA), you will work under the supervision of a licensed Physical Therapist to provide hands-on, one-on-one care to patients in their homes.

This role will primarily serve patients throughout the east Portland metro area (east of the Willamette River) and offers autonomy, flexibility, and meaningful patient relationships.

Key ResponsibilitiesPatient Care
  • Provide physical therapy treatments according to the plan of care established by the supervising PT
  • Assist patients in improving:
    • Mobility
    • Strength
    • Balance
    • Functional independence
  • Observe and report patient responses to treatment
Collaboration & Care Coordination
  • Work closely with Physical Therapists, nurses, and the interdisciplinary care team
  • Communicate patient progress and recommend adjustments to care plans
  • Participate in care coordination and case discussions
Patient & Caregiver Education
  • Instruct patients and families on:
    • Home exercise programs
    • Safety and fall prevention strategies
    • Proper use of assistive devices
Documentation & Compliance
  • Complete accurate, timely clinical documentation
  • Follow Medicare home health guidelines and Oregon state regulations
  • Ensure compliance with company and regulatory standards
Quality & Outcomes
  • Support patient-centered outcomes and satisfaction
  • Help reduce hospital readmissions and promote independence
QualificationsRequired
  • Licensed Physical Therapist Assistant (PTA) in Oregon
  • Graduate of an accredited PTA program
  • Current driver's license and reliable transportation
  • Ability to work independently in a field-based setting
Preferred
  • Home health experience
  • Familiarity with OASIS and Medicare documentation
  • Strong time management and clinical skills
