Position Overview
Home Health Physical Therapist Signature Healthcare at Home is hiring a full-time Physical Therapist to serve the Northeast Portland Metropolitan Area, including neighborhoods such as Alberta, Hollywood, Parkrose, Laurelhurst, Gateway, and surrounding communities. This role is ideal for PTs who thrive in dynamic, community-based care environments.

Key Responsibilities
PT Responsibilities
  • Conduct home-based physical therapy evaluations.
  • Develop individualized treatment plans focused on functional mobility and independence.
  • Deliver therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
  • Perform home-safety assessments and recommend appropriate DME.
  • Manage a field-based caseload with efficiency and autonomy.
  • Collaborate with interdisciplinary team members to ensure coordinated care.
  • Maintain accurate documentation in accordance with regulatory standards.
  • Support safe transitions from hospitals, SNFs, and outpatient settings.

Qualifications
PT Requirements
  • Current Oregon Physical Therapist license (or ability to obtain).
  • One year of PT experience preferred; home-health experience a plus.

  • Reliable transportation and valid driver's license.
