About the Role

We’re looking for a dedicated Hospice Admission Nurse - RN to support patients and families as they enter hospice care. This role is essential in ensuring a smooth, compassionate transition into services.

You will conduct initial hospice assessments, determine eligibility, develop individualized care plans, and guide patients and families through the admission process. You’ll collaborate closely with an interdisciplinary team to ensure care aligns with each patient’s goals, wishes, and needs.

Schedule:
Full-time, Monday–Friday with 8- or 12-hour shift options. Participation in a holiday and monthly weekend rotation is required. Schedule may adjust based on organizational needs.

How You’ll Make a DifferenceInitial Assessments & Care Planning
  • Lead hospice admissions, ensuring a timely and compassionate transition into care

  • Perform initial hospice eligibility and comprehensive RN assessments

  • Assess medical, physical, emotional, and social needs; develop nursing diagnoses

  • Initiate and coordinate individualized care plans with the interdisciplinary team

Patient & Family Support
  • Educate patients and families on hospice services, expectations, and symptom management

  • Facilitate informed consent and complete all admission documentation

  • Provide emotional support to strengthen coping and understanding

  • Advocate for patient goals, dignity, and preferences throughout care

Clinical Excellence
  • Initiate nursing interventions including pain and symptom management

  • Maintain accurate, timely documentation in the EMR

  • Follow all safety, infection control, and clinical standards

Team Collaboration & Coordination
  • Partner with Access Specialists and referral sources to coordinate admissions

  • Communicate effectively with physicians, social workers, chaplains, and care teams

  • Identify needs for additional interdisciplinary support and ensure timely involvement

  • Support continuity of care across home and inpatient settings

  • Cross-train to assist with home care visits and inpatient admissions

Professional Responsibilities
  • Uphold patient confidentiality, autonomy, and dignity

  • Demonstrate strong clinical judgment and independent decision-making

  • Foster a positive, collaborative work environment

  • Maintain a valid driver’s license and travel throughout the service area

What You Bring
  • Active RN license in the state of Virginia (BSN preferred)

  • Graduate of an NLN-accredited nursing program

  • At least one year of clinical experience required (med-surg, oncology, or acute care preferred)

  • Hospice or home care experience preferred

  • Strong assessment, communication, and documentation skills

  • Ability to work independently and collaboratively

Physical & Work Requirements
  • Ability to stand, walk, lift, bend, kneel, and assist patients for extended periods

  • Visual and hearing acuity to monitor patient condition and use medical equipment

  • Ability to manage multiple tasks and interruptions in a dynamic environment

  • Capability to handle emotional stress and support patients and families

  • Ability to travel to patient homes in varying conditions
