Job Description

Company Job Description/ Day to Day Duties:


You provide a deliberate patient and family centered partnership with the healthcare team that facilitates a seamless patient experience between the Inpatient and Outpatient settings as a registered nurse case manager. The Registered Nurse Care Coordinator works to maximize the patient's health outcomes and the patient experience by identifying patient and family needs, completing pre-visit planning, supporting the clinic visit and providing closure to the clinic visits. You work to develop and support a coordinated plan of care between visits to promote a smooth transition for patients and family across the healthcare continuum that instills a sense of trust and patient advocacy. 



Perferred Qualifications:

 BSN or MSN

Knowledge of RN case management

Patient advocacy experience

Previous inpatient care experience

Previous experience providing patient and family centered care and care coordination across the healthcare continuum

Excellent time management, prioritization, multi-tasking and organizational skills

Ability to work in multiple computer systems

Skilled in using EPIC

Critical thinking skills

Ability to provide lifestyle and chronic disease/complex case management services

Ability to build positive relationships

Ability to delegate appropriate tasks while functioning within a team based model

Flexibility, adaptability and teamwork 

Qualifications

Current MN RN license

BLS Certification

1+ years of clinical nursing experience

Proven ability to function at highest level within scope of practice

Patient education and pro-active disease management experience

Excellent assessment and care coordination skills

Experience with an EMR

Keyboarding/computer skills

Strong customer focus to accommodate patients

Proven ability to follow protocol and specific care delivery expectations per sub specialty population
