Certified Nursing Assistant (CNA)

We are seeking a compassionate, dependable, and dedicated Certified Nursing Assistant (CNA) to provide exceptional care and support to individuals in residential and community-based settings. In this rewarding role, you will help individuals achieve their highest level of independence, health, and quality of life while working as part of a collaborative team.
The CNA is responsible for ensuring the health, safety, supervision, and well-being of individuals served in accordance with their individualized care plans. This position provides direct personal care, medication administration, healthcare support, community integration, and assistance with daily living activities. The CNA works closely with the Registered Nurse (RN), Program Director, and interdisciplinary team to maintain a safe, supportive, and person-centered environment.
Key Responsibilities
Direct Care & Support
  • Provide compassionate, person-centered care and supervision to individuals receiving services.
  • Assist with activities of daily living (ADLs), including:
    • Bathing
    • Grooming
    • Dressing
    • Toileting
    • Oral hygiene
    • Eating and meal support
  • Administer medications and prescribed treatments in accordance with physician orders, organizational policies, and regulatory requirements.
  • Assist individuals with mobility needs, including lifting, transferring, positioning, and ambulation support.
  • Implement approved behavioral support strategies and crisis intervention techniques when necessary.
  • Participate in physical interventions as trained and required for CPI and individual support needs.
  • Accompany individuals to medical appointments, recreational activities, community events, and other outings.
  • Provide support in a variety of settings, including residential homes, community locations, medical facilities, and hospitals.
Individual Support & Program Participation
  • Assist in monitoring individual progress, skill development, and environmental factors that affect well-being.
  • Support the implementation of Individual Support Plans (ISPs) and care plans.
  • Maintain required staffing ratios and supervision levels as outlined in ISP requirements.
  • Accurately document services provided, progress toward goals, and significant events.
  • Participate in team meetings, care planning discussions, and program development activities.
Health, Safety & Environment
  • Maintain a clean, safe, sanitary, and welcoming living environment.
  • Monitor for and promptly report safety concerns, maintenance issues, health changes, or incidents.
  • Collaborate with the RN and Program Director to ensure effective healthcare coordination and regulatory compliance.
  • Promote the health, dignity, rights, and independence of individuals served.
Administrative Responsibilities
  • Complete required reports, documentation, and recordkeeping accurately and on time.
  • Perform administrative tasks and other duties as assigned.
  • Attend required trainings, meetings, and professional development opportunities.
Qualifications
Required
  • Current Certified Nursing Assistant (CNA) certification in good standing with the state nursing board.
  • High School Diploma or GED.
  • Valid driver's license.
  • Reliable transportation.
  • Demonstrated safe driving record and ability to transport individuals when required.
