Location: Greenfield, with regular travel to Union City and Yorktown


About the Role

We are seeking a compassionate and organized Social Services Coordinator to provide person-centered support to residents and their families across three communities. This role focuses primarily on Medicaid eligibility, applications, redeterminations, spend-down cases, and Medicaid Waiver programs, while also supporting residents with social services needs.


Key Responsibilities
  • Complete, submit, and track Medicaid applications, redeterminations, and spend-down cases.
  • Coordinate with residents, families, DFR, Maximus, and the Area Agency on Aging.
  • Monitor pending Medicaid cases and follow up on required documentation to prevent coverage lapses.
  • Partner with the Business Office to ensure accurate income, resource, and patient liability reporting.
  • Educate residents and families about Medicaid eligibility, Waiver programs, applications, and appeals.
  • Assist Admissions with Medicaid-related documentation for new and prospective residents.
  • Facilitate resident and family grievances and ensure appropriate follow-up.
  • Maintain accurate, confidential resident records in compliance with HIPAA and applicable regulations.
  • Travel regularly between Greenfield, Union City, and Yorktown.
  • Participate in required meetings, in-services, and training.
  • Perform other duties as assigned.
Qualifications
  • High school diploma or equivalent required; Associate or Bachelor's degree in Social Work, Human Services, or a related field preferred.
  • 3 years of Medicaid application or eligibility experience required.
  • 1 year of Medicaid Waiver experience required.
  • Long-term care or Social Services experience preferred.
  • Valid driver's license and reliable transportation required.
  • Strong knowledge of Medicaid eligibility, applications, redeterminations, and spend-down requirements.
