Job description

We are seeking a qualified Social Worker to assist our patients in our Savannah area. This candidate should be a compassionate Social Worker with excellent customer service skills to assist our patients' multiple facilities and homes in the area.

This role will perform visits in homes and facilities (ALF and ILF's) in their designated service area. You must have reliable transportation as travel is required daily. This is a full time, salary-based working 8-hr shifts Monday-Friday (8am-5pm)

What Are We Looking For?

Your Health is currently looking for a Social Worker to join our growing urgent care family. This role would be working with patients to help complete their advance care planning, cognitive assessments, placement, general resources, etc. The Social Worker would be responsible for the following:

  • Initiating Advance Care Planning for patients throughout their assigned Care Team (including facilities).
  • Completing Daily Comprehensive Assessments on patients.
  • Completing Daily Care Plans on patients.
  • Documenting Chronic Care Management time daily.
  • Meeting Daily Productivity Goals
  • Coordinating discharges at contracted facilities
  • Coordinating Placement.
  • Continuation of coverage (i.e. coordinating acute visits for falls, med changes, etc.)
  • Build and maintain professional relationships with referral sources.
  • Maintain strong communication and professionalism between referral sources, patients/families, and Care Teams.
