Health Dimensions Group is seeking a Director of Wellness (RN) to lead the clinical operations of two CBRF senior living communities within the Dimensions Living Portfolio — Dimensions Living Pewaukee East and Dimensions Living Pewaukee West— located in Pewaukee, Wisconsin.

What You'll Do (AKA: Your Superpowers)Lead Clinical & Resident Wellness
  • Oversee all clinical and wellness services throughout the two communities (60-CBRF bed total).

  • Ensure residents receive exceptional, person-centered care through comprehensive assessments, individualized care planning, and medication management.

  • Promote a culture focused on dignity, independence, safety, and quality of life.

  • Partner with residents, families, physicians, pharmacies, and outside providers to coordinate exceptional care.

Develop & Inspire Your Team
  • Recruit, mentor, and develop nurses and caregivers to build a high-performing wellness team.

  • Foster a positive, collaborative culture focused on accountability, engagement, and professional growth.

  • Provide coaching, education, and ongoing support to strengthen clinical performance and resident outcomes.

Drive Clinical Excellence & Compliance
  • Maintain survey readiness and ensure compliance with all state and federal regulations.

  • Monitor clinical documentation, resident assessments, incident reporting, infection prevention, and medication management.

  • Lead quality improvement initiatives while partnering with pharmacy consultants, physicians, and interdisciplinary teams.

Partner Across the Community
  • Collaborate closely with the Executive Director and department leaders to support community goals.

  • Participate in resident care conferences, family meetings, and interdisciplinary planning.

  • Help cultivate a community where wellness is integrated into every resident experience.

Support Residents & Families
  • Serve as a trusted clinical resource for residents and their families.

  • Build strong relationships through proactive communication, education, and compassionate support.

  • Promote programs that enhance residents' physical, emotional, and social well-being.

What You Bring to the Table (Besides Your Clinical Expertise)Education & Licensure
  • Graduate of an accredited school of nursing.

  • Current RN license in the State of Wisconsin.

  • CPR certification preferred.

  • Strong understanding of infection prevention and senior wellness best practices.

Leadership & Clinical Experience
  • Minimum two years of nursing leadership experience within senior living.

  • Experience leading, coaching, and developing clinical teams.

  • Demonstrated ability to manage resident care, regulatory compliance, and quality initiatives.

Clinical Knowledge & Operational Skills
  • Working knowledge of Wisconsin senior living regulations.

  • Strong assessment, care planning, and clinical decision-making skills.

  • Excellent organizational skills with the ability to prioritize multiple responsibilities.
