Position Overview

Bright Path Social Work is seeking a dedicated and licensed Senior Master Social Worker to join our dynamic team in Colorado Springs, Colorado. This full-time remote position is perfect for experienced social workers who are passionate about improving the lives of seniors and providing guidance to their families. The ideal candidate will have at least two years of proven experience in social work, particularly in geriatric care, and be committed to promoting the well-being and quality of life for our senior community.

Work Environment

This position offers a remote work setting, allowing for flexibility while maintaining a strong connection with clients. Our work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend availability, which may be required based on client needs. You will engage regularly in remote interactions with clients and team members, utilizing technology to coordinate care and services. On-call duties may be necessary to provide timely support for urgent client situations, ensuring our seniors receive continuous care.

Key Responsibilities
  • Conduct thorough psychosocial assessments of seniors, evaluating emotional, mental, and social factors influencing their well-being.
  • Develop individualized care plans in collaboration with clients and their families, ensuring alignment with their specific goals and preferences.
  • Provide counseling and emotional support, assisting seniors in navigating life transitions such as health changes, relocation, or loss.
  • Advocate for clients’ rights and facilitate access to community resources, ensuring they receive appropriate support services.
  • Collaborate with interdisciplinary teams, including healthcare professionals and caregivers, to deliver coordinated and holistic care.
  • Conduct regular check-ins to monitor client well-being, assess living conditions, and intervene in any safety or care issues.
  • Facilitate communication among family members involved in the care of senior clients, helping to resolve conflicts and promote understanding.
  • Educate clients and families about available community resources, support groups, and programs that enhance social engagement.
  • Maintain accurate documentation of client interactions and care plans in our electronic health record (EHR) system, adhering to confidentiality regulations.
  • Stay updated on the latest social work practices, policies, and ethical standards to ensure compliance and best practices.
Patient Population / Client Focus

You will primarily work with seniors, addressing their unique needs that arise from aging, chronic illnesses, and social isolation. Your expertise will contribute significantly to enhancing their quality of life by promoting mental and emotional health, encouraging social interactions, and facilitating access to necessary resources. Understanding the sensitive nature of this demographic is crucial, as many clients may deal with complex issues related to health, family dynamics, or financial concerns.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • Current and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of experience in a social work role, ideally within a community center or geriatric setting.
  • Deep understanding of social work principles, geriatric issues, and available community resources for seniors.
  • Excellent communication skills, both verbal and written, with strong interpersonal abilities to foster relationships.
  • Demonstrated proficiency with electronic health record (EHR) systems and other relevant technology.

Preferred Experience
