The Social Work Assistant (Social Worker I) is a field and office-based position responsible for screening and assessing Home Delivered Meal clients. The Social Work Assistant will work with potential program applicants to provide information, screening, and complete intake and enrollment paperwork for homebound older adults. The position will also assist the Home Delivered Meal Supervisor with administrative duties, quarterly home and phone assessments, and other program components as assigned. This position requires an individual with strong communication, time management, attention to detail skills, and excellent computer and organizational skills. It also requires an ability to interact and provide customer service with individuals at all levels and handle a large volume of client assessments.

QUALIFICATIONS

  • Bachelor’s Degree from an accredited university in human services or two (2) years of experience in homecare or a related field.
  • Bilingual in Russian, Spanish or Farsi preferred
  • Current unrestricted CA Driver’s License and proof of auto insurance.
  • Experience and interest in providing health services to older adults
  • Case management and community-based service delivery experience preferred
  • Knowledge of community resources and the aging process
  • Knowledge of safety issues when working with at-risk clients
  • Experience conducting in-home visits strongly preferred
  • Ability to function independently and with flexibility
  • Experience with Electronic Health Records systems desired
  • Must be computer literate in MS Office applications (Word, Excel, and Outlook)
  • Demonstrated ability to work in an interdisciplinary community setting maintaining appropriate professional boundaries
  • Culturally sensitive and able to work with a multicultural client/customer population; ability to work in a non-

Judgmental manner and foster positive outcomes; able to work as part of a team and maintain professional

boundaries.

  • Excellent communication skills both verbal and written and ability to utilize telehealth platforms.
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

RESPONSIBILITIES/ESSENTIAL FUNCTIONS:

  • Complete initial intake forms, participant agreement forms and level of need assessments for HDM applicants
  • Complete quarterly phone and in-home assessments for all assigned HDM clients
  • Communicate recommendations and referrals for HDM clients as needed
  • Develop assessment and intake quarterly goals in conjunction with HDM Supervisor and effectively carry out assignments in a timely manner
  • Maintain client assessment tracking forms
  • Other related duties as assigned
