Job description

Job Title: Medication Technician

Department: Clinical

FLSA Status: Non-Exempt

Reports to: Wellness Lead (WL) /Wellness Director (WD)

JOB SUMMARY:

The Medication Technician is responsible for maintaining a safe and secure delivery of medications, treatments and health related items to the residents within the Community. The Medication Technician assists in developing, coordinating and performing the tasks on the service plan according to the residentsโ€™ preference and abilities through the nurse's assessment of care. The Medication Technician may serve as a Manager on Duty and monitor all the caregivers on the same shift.

ESSENTIAL FUNCTIONS:

  • Adheres to and conveys the philosophy of the community assisted living/ memory care policies and procedures. Has awareness and practices in accordance with the regulations of the state.
  • Receives reports from the previous shift clinical team as well as providing a thorough report to oncoming shift.
  • Reads the 24-hour report sheet or system for alerts, and shower schedule for information and changes at the beginning of each shift. Documents on 24-hour report sheet or system on new added events, changes in care or conditions as well as Progress notes, as appropriate throughout the day.
  • Treat residents, families and visitors of the Community with dignity and respect.
  • Assists residents with activities of daily living, environmental orientation, vital signs, first aid, and communication with outside health care professionals for resident quality of care.
  • Demonstrates knowledge of infection control and standard precautions.
  • Documents care provided adhering to Community policies and procedures.
  • Maintains a clean, neat, comfortable, safe environment for residents, staff, and visitors including housekeeping duties as assigned. Ensures secured doors are engaged and alarmed at all times, as indicated.
  • Reports deficiencies observed in resident apartments clothing/linen, safety concerns and community to WL, WD and/or Executive Director.
  • Informs WL and WD of any change in resident care in a timely manner.
  • Completes Incident Report form as needed for any unusual occurrence in resident skin, fall, altercation, elopement, found on floor etc.
  • Document alert charting observations in the resident progress notes.
  • Follow up on any medication that is not available per orders to be administered with 3rd party vendors, pharmacy, family, POA, etc., documentation in resident progress notes of all efforts to obtain mediations. Follow with report to WL and WD update on 24-hour report sheet as well as shift report.
  • Observes and communicates any changes in resident physical or mental status as directed.
  • Is thoroughly familiar with fire, missing persons and other emergency procedures, follows Community policies and procedures detailed in fire and disaster plans.
  • Avoids loss, breakage, and waste of supplies and equipment.
  • Maintains a neat and orderly Medication Room.
  • Maintain a clean well organized medication cart, ensuring all meds are properly labeled, stored appropriately, medication is ordered timely, cycle fill is properly maintained.
  • Medication carts are restocked for the oncoming shift, trash removed, spills cleaned.
  • Wears name tag all the time, following the community dress code.
  • Assists residents moving in or out of the community. Include admission note of observations and discharge details in progress notes.
  • Count Narcotics at change of each shift validation of correct count, alert immediately to WL and WD if count is off.
  • Ensuring that every resident has a monthly full set of vital signs and weight or more often as applicable per physician order.
  • Maintains current resident record for each resident including but not limited to obtaining current physician orders, clarifying orders, and alert and monthly charting.
  • Orders, stores and assists with medications according to community policy and procedure and current physician orders.
  • Maintains, transcribes and documents in the Medication Assistance Record (MAR) according to Policy and Procedure.
  • Maintains service plans/care directives. May participate in Service Plan reviews.
  • Supervises and monitors clinical floor staff.
  • Performs all other duties assigned by any member of management.

EDUCATION & EXPERIENCE:

  • Performs resident care with an awareness of dignity and individuality, with understanding and concern.
  • Maintains acceptable attendance record; notifies supervisor if late or unable to work in accordance with personnel policies; follows work schedule.
  • Requires minimal supervision. Is a self-starter.
  • Organizes and utilizes time appropriately; sets priorities and accomplishes assigned tasks.
  • Displays mature behavior and attitude in speech and action; demonstrates consistently pleasant demeanor, tone of voice, supports the philosophy and approach to care used by the Community.
  • Keeps resident information confidential; respects residentโ€™s rights; respects privacy and right to self-determination of residents.
  • Is able to communicate in English effectively, verbally, and in writing.
  • Has ability to spend long periods of feet and to ambulate quickly.
  • Is accurate, dependable, neat and thorough in assigned tasks.
  • Ability and willingness to learn quickly.
  • Ability to utilize and apply basic management skills.
  • State Criminal Background Check and LIC 508 Criminal Record Statement.
  • LIC 503 Health Screening Report.

ANY CERTIFICATION PER STATE REQUIREMENT:

  • Has familiarity with willingness to work with elderly population.
  • Tuberculosis (TB) clearance as required by the State.
  • Criminal Background check cleared.
  • Fingerprint Clearance as required by State.
  • Must be at least 18 years of age as outlined in state regulations.
  • Medication administration certificate (per state requirement).
  • First Aid and CPR Certification as required by State.

PHYSICAL DEMANDS & WORKING CONDITIONS:


Physical Demands:

  • Ability to stand and walk for extended periods.
  • Ability to stoop, kneel, crouch, or crawl as part of regular duties.
  • Ability to talk and hear to communicate effectively with residents and staff.
  • Ability to lift and/or move up to 50 pounds independently.
  • Vision requirements include close vision, distance vision, and the ability to adjust focus.

The following physical actions may be required but are not regularly expected:

  • Sitting
  • Use of hands and fingers to handle or feel
  • Reaching with hands and arms
  • Tasting or smelling
  • Lifting more than 50 pounds (rarely)

Work Environment

  • Regular interaction with co-workers, residents, and vendors.
  • Occasional evening, weekend, or night shifts may be required to ensure proper shift coverage.
  • Subject to potential exposure to:
    • Communicable diseases and infections
    • Physical injury from transferring, repositioning, or lifting residents
    • Possible exposure to blood-borne pathogens.
    • Various drugs, chemicals, infectious, or biological hazards
    • Environmental hazards such as falls, burns, odors, or equipment-related injuries
  • On-call work may be required on an as-needed basis.
  • Occasional travel may be necessary.
  • There is no anticipated regular exposure to latex.
