Acclaim Home Health Registered Nurse (RN)

Acclaim Home Health is seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality, patient-centered care to individuals in their homes. The RN is responsible for assessing patient needs, developing and implementing individualized care plans, and ensuring care is delivered in compliance with physician orders, agency policies, and state and federal regulations.

Key Responsibilities
  • Perform comprehensive patient assessments and ongoing evaluations in the home setting
  • Develop, implement, and update individualized plans of care in collaboration with physicians and the interdisciplinary team
  • Provide skilled nursing services including medication management, wound care, disease management, and patient education
  • Accurately document patient care, assessments, and outcomes in a timely manner
  • Coordinate care with therapists, aides, physicians, and other healthcare professionals
  • Educate patients and caregivers on disease processes, treatments, and self-care
  • Ensure compliance with Medicare Conditions of Participation, state regulations, and agency policies
  • Participate in quality improvement activities and case conferences as required
Qualifications
  • Current and active Registered Nurse (RN) license in the state of practice
  • Minimum of [1 year] clinical nursing experience (home health experience preferred)
