Heartline Staffing is recruiting an experienced Registered Nurse Case Manager for a full-time Senior Care Options (SCO) opportunity in Lynn, Massachusetts. This position offers an exceptional work-life balance with a hybrid schedule of three days in the community and two days remote, Monday-Friday daytime hours, and no nights, weekends, or holidays.
The RN Case Manager provides comprehensive case management for assigned SCO members with complex needs, coordinating cost-effective, quality care through individualized assessment, care planning, implementation, and ongoing evaluation.

POSITION OVERVIEW
The RN Case Manager maintains responsibility for an assigned caseload of SCO members with complex healthcare needs. The nurse develops, implements, and evaluates individualized plans of care while coordinating with physicians, members, families, health team members, Commonwealth Care Alliance (CCA), and the broader provider community.
CASE MANAGEMENT RESPONSIBILITIES
Complete comprehensive assessments of member needs, condition, and environment
Develop individualized plans of care corresponding with physician orders
Regularly evaluate and update patient care plans as needs change
Maintain ongoing responsibility for an assigned caseload
Oversee implementation and evaluation of individual plans of care
Ensure delivery of competent, quality patient care in the home
Coordinate care with physicians and interdisciplinary health team members
Maintain communication with Commonwealth Care Alliance (CCA)
Collaborate with the provider community as appropriate
Work directly with members and their families
Provide patient education
Maintain required case-management documentation
Participate in team knowledge-sharing activities
COMMUNITY-BASED CARE
This is a community-focused case management position requiring frequent local travel. The RN works with members and families in their home environment while coordinating services and care across the healthcare continuum.

Minimum 3 Years of Licensed Registered Nurse Experience REQUIRED

Ability to Travel Locally on a Frequent Basis REQUIRED

Proficiency with Microsoft Office Tools REQUIRED
Bachelor of Science in Nursing PREFERRED
5 Years of RN Experience PREFERRED
Spanish Speaking PREFERRED
COVID Vaccination PREFERRED
RN EXPERIENCE: A minimum of three years of licensed RN experience is required. Five years is preferred. Do not reject a candidate who has at least three years solely because they do not have five.
HOME CARE EXPERIENCE: The supplied qualifications specifically include home care clinical experience. Verify actual hands-on home/community-based clinical experience during discovery rather than assuming hospital case management is equivalent.
EDUCATION: A Bachelor of Science in Nursing is preferred, not required. Do not reject an otherwise qualified ADN or diploma-prepared RN solely for not holding a BSN.
SPANISH: Spanish-speaking ability is preferred, not required for this Lynn SCO position.
HYBRID / TRAVEL REQUIREMENT: This is not a fully remote position. The structure is three days in the community and two days remote, and the RN must be able to travel locally on a frequent basis.
