POSITION: School Nurse -RN/LPN

REPORTS TO: Building Principal/District Nursing Coordinator

LOCATION: Abraham Lincoln Elementary

JOB DESCRIPTION:

Maintain a professional relation with staff, students, and parents while preserving confidentiality of All medical information. Provide acute care for student(s). Keep documentation of student care, follow physician'sโ€™ orders while caring for students, administer routine and PRN medications as directed. Implement District Wide School Health Services Program

REQUIRED QUALIFICATIONS:

ยท Graduate from a diploma school, associate or baccalaureate degree from an accredited institution or higher learning.

ยท Valid License from the State of Indiana to practice as a Registered Nurse or Licensed Practical Nurse

ยท Valid CPR Certification Card

DUTIES and RESPONSIBILITIES:

1. Collects information about the health and developmental status of the student and uses this data to determine a nursing diagnosis.

2. Implements a plan of care, under the direction of a physician, with specific goals and interventions that are unique to student needs.

3. Administers medication and treatment prescribed by physicians

4. Conducts parent conferences and maintains liaison between physicians, parents and staff

5. Make recommendations to primary care physician on health needs of individual students

6. Maintains up-to-date cumulative health records on all students requiring nursing intervention

7. Report to principal any potential health and contagious disease situations for concern of student(s) and staff.

8. Reports to parents, school personnel, physicians, clinics and other agencies on student health matters.

9. Administers first aid in accordance with established first aid procedures as defined by the Nurse Practice Act.

10. Implements board policy on exclusion and readmission of students in connection with infectious and contagious diseases.

11. Collaborates with other professionals in planning to assure quality of health care provided to students

12. Utilizes the RDS System to input and review student health information

13. Maintains daily, monthly and yearly statistical information regarding health status of students and staff and utilization of school health services programs.

14. Implements all screening programs mandated by Indiana Law and complete on-line state reports.

15. Supervise and mentor nursing students from local universities.

16. Reports all accidents and completes the necessary accident reports

17. Completes Workmanโ€™s Compensation forms for employees with work-related injuries

18. Provide educational programs for students (i.e. asthma, personal hygiene, nutrition, and hand washing)

19. Supports and follows the Indiana Standards for School Nursing Practices.

20. Any and all duties assigned
