Job description

Scheduling Coordinator/Scheduler manages the schedules /assignment of all Certified Home Health (CHHAs) staff services and disciplines ( Staff/Contractual), to ensure timely and consistent home care for patients. They coordinate schedules, manage staffing issues, and communicate with clients, family members, caregivers and providers.

In essence, the CHHA Scheduling Coordinator acts as a central point of contact for scheduling and staffing, ensuring that patients receive the care they need in a timely and efficient manner.

KEY ROLE/ RESPONSIBILITIES:

Scheduling/Staffing:

· Plan and coordinate CHHA visits, ensuring adequate coverage and timely care for patients.

· Address scheduling conflicts, manage no-shows, and resolve staffing issues.

Communication and Record Keeping:

· Communicate with patients, families, caregivers, and other team members regarding schedules and changes.

· Maintain accurate records of patient visits, care plans, and scheduling adjustments in systems like, Netsmart, HHA Exchange, admissions log, Productivity log

· Ensure appropriate assessments and visits are scheduled and assigned- 30 days rehab reassessment, recert, ROC/SOC and HHA supervisions

· Maintain agency productivity logs and communicate progress with DPS/Staff.

Coordination/ Problem Solving:

· Collaborate with other team members, including nurses and case managers, to ensure smooth patient care.

· Identify and resolve scheduling issues, ensuring that patients receive timely and appropriate care.

· Providing information to clients, providers, referral sources, staff, and the public

· Answering and transferring telephone calls or taking messages.

· Working with other members of the healthcare team, such as nurses and therapists, to ensure coordinated care for clients

General Office Support:

· Assist other staff as and when needed

· May involve sorting and delivering incoming mail, or other administrative tasks.

  • Customer Service:

· Provide excellent customer service to patients, families, and caregivers.

Skills and Qualifications:

  • Strong communication and interpersonal skills.

  • Ability to multi-task and prioritize tasks in a fast-paced environment.

  • Familiarity with home care scheduling software such as: Netsmart, HHA Exchange, etc.

  • Knowledge of home care regulations and policies.

  • Proficiency in Microsoft Office and other relevant software.

  • Experience in a home care setting is often preferred.

  • Basic computer skills.

  • Experience with high volume coordination

  • Flexibility with working over-time as needed

  • Home care experience/CHHA

Schedule/ Work hours:

  • 7.50-hour shift – 9:00 AM – 5:00PM

  • Work Location: In person
