Outpatient Care Leader
Job Description
Outpatient Care Leaders perform at a competent level of nursing practice and according to the appropriate standards of practice for the area. Coordinates patient care and service through a multidisciplinary team approach. Organizes the daily operation of the team and following up to be certain that the work is accomplished. Creates an environment of caring and concern for patients and staff members according to the Customer Service principles. This position will have supervision of the clinical and non-clinical staff in the department and report directly to the clinic director.
Department: Infusion Center
Hours: Full time, 40 hours/week
Shift: 1st
Required: ADN, BSN, RN License in State of IL, BLS and ACLS certification within 90 days of hire
Pay: Based on experience, starting at $85,488.00
Responsibilities
Assists with the development and interpretation of policies and procedures., Assists with unit activities for personnel from outside agencies., Assures all new employees are appropriately oriented and trained. Ensures compliance with all regulatory agencies., Collaborates with physicians and other members of the health care team to address issues related to patient population through the process of mutual information sharing, discussion, and decision-making., Communicates patient care issues and concerns in an appropriate forum., Develops in conjunction with the patient care team an appropriate and timely plan of care for the assigned caseload of patients., Ensures the availability of supplies needed., Evaluates other team member as assigned using established criteria and goals,. Participates in peer review process., Identifies learning needs of staff on team. Coordinates annual competency assessment. Follows-up with staff to assure standards are met. Initiates, develops, and presents educational program/activities (formal/informal)., Implements the role of preceptor according to Preceptor Accountabilities., Initiates the development and/or revision of standards appropriate to unit., Interacts with co-workers to assure a smooth flow of patients., Interacts with patients, physicians, and staff to resolve issues. Provides clinical expertise to clinical staff in regard to patient care., Maintains and follows all clinical policies and procedures., Participates, develops and implements performance and/or quality improvement initiatives., Participates in the ongoing variance monitoring of clinical and implements action plans based on results of quality improvement activities, which impact on patient care outcomes., Performs RN duties as defined in the RN job description., Provides consultation and support to those who plan and administer nursing care., Provides daily supervision and management of clinical and non-clinical staff members. Hire, trains, supervises staff, evaluates work and recommends and implements approved corrective actions., Provides rationale for patient care decisions based on in depth knowledge and experience of patient population., Responsible for scheduling staff., Responsible for the direct supervision of personnel and nursing activities., Serves as a resource for clinical skills to staff. Evaluates staff performance of clinical skills., Works to maintain efficient flow of department activity, verifying physician orders, scheduling, screening of patient's prior authorizations, patient education, etc.
