Hospice Nurse Practitioner Position:

Are you a compassionate, skilled clinician who feels called to make a difference in the lives of patients and families facing end-of-life care? Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking aย Hospice Nurse Practitioner to join our dedicated team.

As a valued member of our hospice care team, youโ€™ll provide expert clinical support, symptom management, and compassionate guidance to patients and their loved ones. Youโ€™ll collaborate with physicians, nurses, social workers, and chaplains to ensure comfort, dignity, and quality of life are always at the heart of what we do.

What Youโ€™ll Do as a Hospice Nurse Practitioner:

  • Provide hospice patient assessments, symptom management, and clinical direction in collaboration with the hospice medical director.
  • Work closely with the interdisciplinary team to create and adjust individualized plans of care.
  • Offer education and emotional support to patients and their families.
  • Participate in team meetings and ongoing quality care initiatives.

Who you are as a Hospice Nurse Practitioner:

Current Nurse Practitioner license (CRNP) in the state of Pennsylvania.

  • Hospice and/or palliative care experience preferred.
