LVN Hospice Liaison / Hospice Sales


Marin County Territory

Monday – Friday | 8:00 AM – 4:30 PM


A growing hospice organization is seeking a compassionate and motivated LVN/LPN Hospice Liaison to join its expanding team in Marin County. This is an excellent opportunity for a hospice clinician looking to transition into a relationship-focused liaison and healthcare sales role while continuing to make a meaningful impact on patients and families. The ideal candidate will have prior hospice experience, strong interpersonal skills, and a passion for educating patients, families, and referral partners about hospice care and services.



Position Overview

As an LVN/LPN Hospice Liaison, you will partner closely with the hospice sales and clinical teams to support referral development, patient admissions, and community outreach efforts throughout the territory. This role blends clinical knowledge with relationship-building and marketing responsibilities to help grow hospice awareness and support patients during critical moments of care.

You will serve as a trusted resource to referral partners, patients, families, and internal teams while helping drive census growth and exceptional customer service.


Responsibilities

  • Support Hospice Liaisons and clinical teams with patient referrals, admissions coordination, and community outreach initiatives
  • Build and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, case managers, and other referral sources
  • Educate patients, families, and healthcare professionals on hospice philosophy, services, and eligibility
  • Conduct introductory meetings with patients and families and assist with hospice consent discussions
  • Participate in community events and brand awareness initiatives throughout the service area
  • Maintain accurate CRM documentation, activity reporting, and referral tracking
  • Identify referral opportunities and communicate market feedback, trends, and growth opportunities to leadership
  • Serve as a hospice resource for referral partners and internal team members


Qualifications

  • Current unrestricted LVN or LPN license required
  • Minimum of 1 year of hospice experience required; 2–3 years preferred
  • Prior customer service, healthcare sales, or liaison experience strongly preferred
  • Strong communication and relationship-building skills
  • Comfortable working in both clinical and community-based settings
  • Self-motivated with strong organizational and follow-up abilities
  • Reliable transportation, valid driver’s license, and current auto insurance required
