Job description

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking two (2) Registered Dietitians- Rural Health.

Qualified applicants should reside in one of the following areas:

  • Litchfield County, CT
  • Windham County, CT
  • New London County, CT

The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Primary Responsibilities:

  • Under the direction of the Care Management (CM) Manager of Rural Health Transformation, provide care coordination support for eligible HUSKY Health members residing in rural areas.
  • Conduct targeted outreach to rural members to complete nutritional assessments and collaborate with CHNCT’s Care Management (CM) clinical teams to support the development of person-centered care plans.
  • Serve as a program coach, supporting members participating in virtual health programs by providing individualized guidance, facilitating goal setting, encouraging engagement, and monitoring progress toward program objectives.

Tasks Performed:

  • Conducts member outreach and performs nutrition assessments to identify nutrition and health-related needs, develops interventions to address those needs in collaboration with other team members, and evaluates progress towards the established goals.
  • Promotes healthy lifestyle and dietary habits through coaching and support.
  • Provides viable food and supplement options, and guidance on choosing the healthiest nutritional options to promote adherence to their physician recommended diet.
  • Serves as a program coach by guiding participants through the online educational curriculum, providing personalized coaching via the digital platform, monitoring participant progress, and providing feedback.
  • Presents at virtual member-facing learning groups on program-related topics.
  • Participates in interdisciplinary case rounds/collaborative meetings as appropriate with other members of the Rural Health Transformation team.
  • Follows established escalation protocols when clinical risks are identified.
  • Reviews member educational materials related to nutrition for the CM program and develops nutrition-focused member education materials as needed.
  • Participates in staff education and quarterly peer review/audits as needed.
  • Performs other duties as assigned.

Essential Functions:

  • Engages with members through telephonic, videoconference, and in-person/face-to-face outreach.
  • Utilizes the digital platform and coaching dashboard to facilitate member participation in online learning curriculums.
  • Participates in virtual member learning groups and organized community activities upon request.
  • Maintains knowledge of covered benefits for CT Medicaid and HUSKY Health programs.

Desired Education: Four years post-secondary schooling

Desired Degree: Bachelor's degree

Desired Major: Nutrition Science

Desired Job Experience: Two years of clinical experience preferred.

Other Qualifications: Excellent written and verbal communication skills. Collaborative and team-focused perspective. Bilingual/bicultural abilities a plus. Prefer live in one of the designated geographic locations in need. Current active and unrestricted CT Dietitian/Nutritionist License, Active Registered Dietitian Nutritionist (RDN) Certification by the Academy of Nutrition and Dietetics. Must hold a valid and unrestricted motor vehicle operator’s license and have reliable transportation.

Certifications Required: CT Dietitian/Nutritionist License, State Motor Vehicle License, Registered Dietitian Nutritionist
