Compensation: $27.47 an hour Non-ExemptĀ 

Work Schedule:Ā Monday - Friday 9 AM - 5 PMĀ

Position Summary:

The Rapid Rehousing Case Manager is a key part of the Housing Plus RRH program.Ā The case manager carries a caseload of families, individuals, or a combination of both,Ā and will deliver services to support the goals of housing stability, family stabilization,Ā independent living, and financial security.

Duties/Responsibilities:

  • Conduct initial intakes and assessments and annual assessments as required.
  • Develop initial Individual Service Plan (ISP) including relapse prevention plans with all caseload tenants. Conduct quarterly ISP reviews and annual ISP updates with caseload tenants as required.
  • Provide case management, counseling sessions and support services as required.
  • Refer tenants to other agencies, community service providers and resources as needed.
  • Collaborate with providers to ensure compliance.
  • Assist families and/or individuals with achieving goals of self-sufficiency and independent living.
  • Work with individual and/or the entire family to improve family functioning.
  • Maintain/update case notes and other data in AWARDS database as required. Document all interactions with tenants in AWARDS within 72 hours.
  • Ensure progress notes provide accurate and detailed notes on needs, challenges and barriers, and progress and successes for all family members.
  • Maintain tenant case files that are organized and easy to navigate.
  • Ensure case files include AWARDS and supporting documentation for all family members as required.
  • Maintain housing goal of zero rental arrears for each family and/or individuals.
  • Provide emergency and crisis interventions.
  • Conduct monthly home visits as required for entire caseload.
  • Attend program meetings, agency-wide meetings, and staff training.
  • Perform program objectives in accordance with NYC CoC and allĀ protocols according to HousingPlus and government funder policies andĀ procedures and requirements.
  • Other tasks as assigned.
REQUIREMENTS:

Required Skills/Abilities:Ā 

  • Ability to work onsite 5 days per week
  • Experience working with justice-impacted populations, preferably working withĀ women experiencing homelessness histories, behavioral health needs, and/orĀ criminal legal history.
  • Solid knowledge of case management and motivational interviewing.
  • Proficiency in using Ā Microsoft Office, AWARDS, and Google Meet
  • Good organizational, interpersonal communication, and writing skills
  • Ability to work as a team member.
  • Ability to work as a team member.
  • Familiarity with Human Resource Administration (HRA) policy and procedures.
  • Excellent organizational, interpersonal communication, and writing skills.
  • Proficient computer skills including Microsoft applications (Outlook, Work, Excel, PowerPoint) and Google Suite.
  • Must possess the ability to format and work within spreadsheets and Word Documents.Ā 
  • Experience using case management databases preferred.
  • Some evening/weekend hours may be required.

Education and Experience:

  • Associate or Bachelor's in Human Services or Social Services or related field
  • Minimum of 2-3 years' experience working with families around the issues of homelessness, Mental Illness and/or Substance Use Disorder.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
