Job description

Clinical Lead - Outpatient Assisted Living
Independence Rehab - Rose Villa Senior Living | Portland OR
Full-Time | Flexible Schedule | Therapist-Owned
Helping People Stay Independent in the Later Chapters of Life
Independence Rehab is seeking a compassionate Clinical Lead who finds purpose in helping older adults regain mobility, confidence, and dignity - while leading a therapy team that delivers meaningful outcomes every day.
This role blends clinical practice with leadership. You'll shape team culture, support clinicians, and partner with facility leadership to ensure therapy services remain patient-centered, compliant, and sustainable.
Whether you're an experienced manager or a strong clinician ready to grow into leadership, Independence Rehab is a place where leaders feel supported and teams feel the difference.
What a Day Looks Like
As Clinical Lead, your work is relationship-driven, collaborative, and highly impactful.
You'll lead a medically complex geriatric therapy program while maintaining an active patient caseload.
Your responsibilities include:
• Overseeing the entire therapy department including full-time, part-time, and PRN staff
• Attending multi-disciplinary facility management meetings
• Managing relationships with facility leadership and administration
• Interviewing and hiring therapy staff in partnership with Recruiting
• Creating and managing therapy team schedules
• Monitoring department revenue and expenses to support sustainable profitability
• Serving as a key contact for patients, families, and caregivers
• Carrying a regular caseload (minimum ~50% productivity; varies by program size)
• Ensuring strict compliance with regulatory requirements and documentation standards
• Training and mentoring staff on policies, procedures, and clinical expectations
• Supporting a culture focused on functional outcomes and team collaboration
Why Leaders Choose Independence Rehab
We're a therapist-owned company that operates therapy services independently - allowing clinical leaders to lead thoughtfully without constant external pressure.
Our leaders often say the biggest difference is the balance between autonomy and support.
• Realistic productivity expectations for leaders
• Strong corporate involvement without micromanagement
• Stable leadership with low turnover
• Autonomy in clinical and operational decision-making
• Protection from unrealistic facility pressure
• Therapy operates independently from nursing management
• Strong outcomes and respected reputation
• Opportunity to build and shape your team culture
Leadership & Support
We don't believe leadership should feel isolating.
You'll receive:
• Structured onboarding into DOR responsibilities
• Operational and compliance support
• Recruiting partnership for staffing needs
• Mentorship from experienced therapy leadership
• Ongoing guidance on PDPM strategy and department performance
Benefits
We invest in our leaders in ways that support both career and life:
• Competitive compensation structure
• Up to 3.5 weeks PTO annually
• Comprehensive medical, dental, and vision insurance
• Continuing education reimbursement (up to $750/year)
• Health Savings Account options
• Accident, Critical Illness, and Hospital Indemnity Insurance
• Basic Life and Accidental Death coverage
• Voluntary Life and Disability options
• 401(k) with employer match
• Employee Assistance Program
• Teladoc access
Education & Experience
• Current Washington State Physical Therapist Assistant in good standing
• Minimum one-year SNF clinical experience
• Prior DOR experience preferred but not required
• Solid understanding of Medicare, Medicaid, private pay, and PDPM reimbursement
• Driven, teachable, detail-oriented, and organized
• Competent and confident leading, motivating, and developing others
• Sincere love for the geriatric population - they are who matter most
If you're a clinician ready to lead, build a team, and create a therapy program where both patients and therapists thrive, we'd love to talk.
Apply today - we're actively interviewing.
