RN Case Manager โ€“ Case Management

Location: Portsmouth, NH
Shift: Days โ€“ 8:00 AMโ€“4:00 PM
Schedule: Day shift with weekend rotation
Guaranteed Hours: 36 hours/week
Contract: 13 weeks
Patient Ratio: 1:18

Responsibilities
  • Provide RN case management for acute-care hospital patients.
  • Perform utilization management and review patient needs.
  • Coordinate care with physicians and the multidisciplinary healthcare team.
  • Develop and coordinate discharge plans.
  • Assist with care coordination and appropriate resource utilization.
  • Support revenue integrity processes.
  • Monitor patient progress and facilitate transitions of care.
  • Provide leadership and guidance within the case management team.
  • Document case management activities using Midas.
Requirements
  • Active RN License โ€“ Required
  • Acute Hospital RN Case Management experience โ€“ Required
  • Critical Care experience โ€“ Required
