Registered Nurse (RN) I - Hospice

Schedule: Monday - Friday 8am-4:30pm, back-up on call rotation, holiday on call rotation

Territory: Lafayette IN area

Summary: A Registered Nurse (RN) I - Hospice provides and plans professional nursing care to patients under the direction of a physician. This role involves delivering direct patient care, assisting with examinations, accurately documenting patient conditions, vital signs, diagnoses, and prescribed treatments. The Level 1 RN also administers treatments and medications as directed, contributing to the comfort and symptom control of hospice patients.

Key Responsibilities:

  • Provides skilled nursing care in a patient's home as ordered by the attending physician
  • Observes and monitors patient conditions
  • Performs assessments to develop an individualized plan of care and adjusts as needs change
  • Administers medication as prescribed by the physician
  • Manages multi-disciplinary care as applicable
  • Promotes continuity of care with appropriate admissions, transfers and discharges
  • Counsels patient and family on the disease/injury processes and how to manage
  • Incorporates patient and family in development of plan of care
  • Listens to patient and family members
  • Oversees and supervises total care of patient provided by Aides and LPNs
  • Evaluates the treatments and medical condition of the patient according to the plan of care
  • Other duties as assigned

Qualifications:

  • Performs work under minimal supervision. Handles complex issues and problems and refers only the most complex issues to higher-level staff.
  • Possesses comprehensive knowledge of subject matter.
  • 0 to 2 years of experience

Education and Credentials:

  • Valid RN license in the state(s) where service is provided
  • Bachelor's degree in nursing or healthcare-related field preferred
