Job Summary:

  • The Fellowship is a 12-month long position which combines mentorship, problem based learning, and supported clinical experience in an outpatient family practice setting

  • Under the mentorship of Faculty clinicians the Fellows gradually increase the number and complexity of patients seen

  • Specialty, Women’s Health, and Pediatric rotations add to experiential foundations in Family Practice

  • Salary and full benefits offered

Timeline:

• 5 Positions are available each year

• Application opens in November

• Deadline for submitting completed application and all required criterion is March 31st

• Interviews are scheduled following the closing date

• Candidates will receive an automated e-mail after submission of application

• Applicants are notified of admission decision late spring

• Orientation starts around Labor Day

Criteria:

Applicants will be expected to submit documentation requirements for consideration into the Advanced Practice Registered Nurse (APRN) Primary Care Fellowship.

The following items are required to complete the application process for the Peak Vista Community Health Centers Advanced Practice Registered Nurse (APRN) Primary Care Fellowship Program. All exceptions are at the discretion of the Program Director.

  • Completed PVCHC APRN Primary Care Fellowship Application

  • Must be graduates (by orientation date -- with less than 750 hours of supervised prescriptive practice, as defined by Colorado DORA) or transitioning FNPs who demonstrate an intent to practice as a primary care provider

  • Must have at least 2 years of RN work experience

  • Certification in Primary Care specialty

  • Active & Unrestricted Colorado RN & APRN license (eligible and complete by orientation date)

  • Three (3) letters of recommendation

  • At least one (1) from a clinic preceptor

  • At least one (1) from a work supervisor

  • No peer references will be accepted.

  • Transitioning Nurse Practitioners will be considered on an individual basis

  • Ability to commit to 12-month, full-time program

  • Submit a 500-word essay addressing each of the following topics and submit by email to APRNFellowship@peakvista.org:

  • Describe your work history and how it has prepared you to be a family nurse practitioner.

  • Share a specific example (professional, academic, or personal) of a time you assisted people from an underserved population.

  • What are your professional values as a nurse practitioner?

  • Discuss your expectations and goals of the Fellowship.

  • Explain how you will contribute to the teaching and learning environment of Fellowship and why you are an ideal candidate for this position.
