Pay Range $53.71 - $60.00 Per Hour (Depending on Years of Experience)

Centurion is proud to be the provider of comprehensive healthcare services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Full-Time RN Discharge Planner to join our team at BCBIC (Baltimore Central Booking & Intake Center) located in Baltimore, Maryland.

The RN Discharge Planner is responsible for coordinating patient discharge planning from a correctional setting. The Discharge Planner will work under the direction of the Director of Nursing in collaboration with a multidisciplinary team to develop and implement a safe and appropriate discharge plan to promote continuity of care after release.

Qualifications

  • Current Registered Nurse in the state of Maryland or the ability to obtain license required
  • Current BLS and CPR certifications required
  • On-year experience as a Discharge Planner or Case Manager required
  • Experience in corrections or an acute setting preferred
  • Ability to obtain a security clearance to include drug screen and criminal background check
