Nature of Work
This role involves providing regional oversight for medical case management under the statewide Ryan White Part B Program. Core responsibilities include performing comprehensive patient assessments, developing individualized care plans, and tracking key clinical indicators to optimize treatment outcomes. The position ensures clients seamlessly access primary, specialty, behavioral, and dental care, as well as essential supportive services and medication assistance programs like AIDS Drug Assistance Program (ADAP). A major focus of the position is maintaining rigorous documentation and program compliance with federal, state and Health Resources and Services Administration (HRSA) guidelines. Key administrative duties include managing accurate data entry within tracking systems like CAREWare, conducting regular chart reviews, and quality improvement initiatives. The role requires generating regular performance and summary reports to ensure regional services maintain high standards of care and meet payor of last resort requirements. The position acts as a clinical resource and collaborative partner, building relationships with healthcare providers, pharmacists, social workers, and community organizations to ensure continuous shared care delivery. Responsibilities include developing and distributing educational materials regarding HIV care, treatment options, and risk reduction to both patients and healthcare personnel. The role requires maintaining active involvement in progressional development to stay current on evolving clinical guidelines and regulatory standards.

Minimum Qualifications
Training: Bachelor's degree in nursing from an accredited college or university and four years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an Associate degree in nursing from an accredited college or university or a diploma nursing program and six years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Master's degree in nursing from an accredited college or university.
Special Requirement: Current West Virginia license or temporary permit to practice as a registered professional nurse.
