Job description

Summary

Utilizing the standards of professional clinical practice, the PPG Ambulatory Care RN Lead provides high-quality, evidence-based care across the lifespan to enhance patient safety, reduce adverse events, and improve patient satisfaction. Also supports and promotes optimal health status and helps to manage cost of care within continually expanding, diverse, and complex patient populations. Interacts with patients during face-to-face encounters or through digital communication, establishing long-term relationships. Applies the ANA Code of Ethics for Nurses to their own professional practice. Supports the practice through participation in peer interviews, co-worker training, performance assessments, and serves as the primary clinical resource to other co-workers. Responsible for working collaboratively with leadership, providers, co-workers and members of the extended care team (Nurse Navigator, Licensed Clinical Therapist, and Pharmacist) to provide excellent service to our patients.

During each encounter, the PPG Ambulatory Care RN Lead focuses on the quality of nursing care by applying appropriate nursing interventions. Independently plans and provides professional nursing care for patients in accordance with physician orders or utilizing protocol-driven care. Performs procedures not limited to: EKG, Retina Scans, Suture Removal, BP Checks, Dressing Changes, UTI check, POCT.

Contacts patients, or delegates contact, regarding lab/test results. Identifies and clarifies patient needs. Educates patients and families on conditions, self-management and preventative care. Co-manages care team in-basket. Functions as a mentor and lead care team trainer. Assists with Quality Control for Point of Care Testing.

Education

Must be the graduate of a School of Nursing, BSN preferred.

Licensure/Certification

Must have current licensure as a Registered Nurse. CPR certification required.

Experience

Minimum two years’ experience as a registered nurse, ambulatory experience preferred. Leadership skills and experience with autonomous critical thinking and working in a team-based care environment.

Other Qualifications

Medical terminology and pharmacology background needed. Essential computer skills and knowledge of electronic medical records recommended. Good verbal and written communication skills required.
