Job Summary: The Home Health RN Case Manager is responsible for the overall coordination, management, and delivery of skilled nursing services to patients in their homes. The RN Case Manager ensures that care is provided according to the plan of care, regulatory requirements, and agency policies, while promoting optimal health outcomes, patient independence, and avoidance of unnecessary hospitalizations.

Requirements:

* Current, active Registered Nurse (RN) license in Arkansas.
*Minimum of one (1) year recent clinical experience in acute care, home health, or related setting.
*Knowledge of OASIS and Medicare home health regulations preferred.
* Current CPR certification.
*Valid driver's license, auto insurance, and reliable transportation required.
