So, What's the Job All About?

Manage and oversee the development, implementation, and monitoring of each assigned client's Individualized Service Plan (ISP) and ensure agency documents are in compliance with regulatory standards. The Training Supervisor is responsible for overall program coordination, supervision, development of program activities, and completion of discharge summaries upon a client's termination from UCPH. Also responsible for managing, overseeing, and completing performance evaluations on assigned direct reports.

The Supervisor reports to the Manager of Community Services for direction and guidance of the day-to-day operations of the program. And is directly responsible for the development, implementation, monitoring, and reporting of 15-20 clients’ support plans. In addition, the Training Supervisor is responsible for implementation and reporting of all assigned outcomes for each client assigned to their caseload. The training supervisor will be assigned up to 7 Direct Support Professionals and will be responsible for monitoring their performance, benefit accruals, timeclock entries, training compliance, etc. The Training Supervisor is responsible for implementation and follow-through of activities for the staff and clients that they directly oversee through coordination with team members and peers. The Training Supervisor is responsible for scheduling group activities and community outings for clients assigned to their caseload.

ESSENTIAL JOB FUNCTIONS

  • Ensure that all assessments, annual paperwork, ISPs, data collection measures and progress reporting are completed accurately, and within the Agency designated time frames, by participating in these processes and thoroughly monitoring the work completed by the Direct Support Professionals.
  • Participate in advocacy efforts to ensure assigned clients receive additional services, equipment and support necessary to lead a healthy productive life.
  • Develop and implement quality functional outcome objectives for each client by evaluating each client according to personal preferences, areas of improvement, and support needs.
  • Maintain cooperative, professional working relationships with all clients, families, Agency employees and professionals outside the Agency by demonstrating appropriate conduct, responding promptly to issues or telephone calls, maintaining confidentiality and by other formal and informal communications.
  • Ensure that CARF standards are being met to maintain accreditation across the agency.
  • Provide effective programmatic supervision of assigned employees by monitoring performance, providing feedback directly, and promptly addressing issues within the Training Supervisor's control or referring performance issues to the Manager of Community Services, if appropriate.
  • Must be physically able to assist individuals who may weigh in excess of 150 pounds using proper transferring technologies and/or mechanical lifts.
  • Ability to adhere to all policies and procedures established by the employer and other entities.
  • Use the EHR (Electronic Health Records) system to review and sign off on records daily.
  • Respond in a timely manner when concerns arise to all support coordinators, family members, guardians, and clients.
  • Support the Manager of Community Services in the scheduling and implementation of all community integration initiatives and program activities.
  • Assist in the transportation of clients to community outings, including driving vans, overseeing van documentation, and assisting in tie-downs and safety practices.
  • Follow all procedures regarding to HR practices and progressive disciplinary actions in regard to hiring and managing direct reports.
  • Maintain departmental regulations by ensuring staffing ratios and filling in as needed.

MINIMUM EMPLOYMENT STANDARDS

  • Two years’ experience in Person Centered Planning/ISP development, implementation, and monitoring.
  • Two years supervisory experience, including but not limited to, supervision of employees, implementation of operating policies and procedures, employee performance evaluation and disciplinary procedures, and staff training and development.
  • Experience in adapting teaching strategies and materials to individual learning levels.
  • Ability to prioritize work assignments, schedule time, and meet deadlines.
  • Effective oral and written communication skills.
  • Ability to lift and transfer adult clients using proper assistive devices (individuals who may way more than 150 Pounds).
  • Basic skills with Microsoft Word, Outlook, and Excel.
  • Must secure and maintain all required training according to agency policy.
  • Valid Driver’s License and Clean Driving Record
  • Clean Family Care Safety Registry and National Criminal and Motor Vehicle Report
PREFERRED EMPLOYMENT STANDARDS
  • Bachelor’s degree from an accredited college or university in a related field or equivalent work-related experience.
  • 5+ years of supervisory experience, including but not limited to supervision of employees, implementation of operating policies and procedures, employee performance evaluation and disciplinary procedures, and staff training and development.
  • Experience with an EHR (electronic health record) system


THIS IS A FULL TIME POSITION--MONDAY THROUGH FRIDAY 8:15am to 4:15pm.
